01 — About mannitol

Not sugar. Smarter.

Mannitol is a sugar alcohol (polyol) found naturally in manna ash, seaweed, mushrooms, celery and olives. Industrially it is made by hydrogenation of fructose or extracted from seaweed. White, crystalline, with a clean cool sweetness and a unique superpower: it barely absorbs water and barely raises blood sugar.

Unlike sucrose, mannitol is not fermented by oral bacteria — so it does not cause cavities. Unlike sorbitol, it does not pick up moisture from air — so tablets stay hard, chewing gum stays crunchy, and powders stay free-flowing for years. In hospitals, 10-20% mannitol infusions pull water out of swollen brain tissue and into kidneys. One molecule, three super-roles.

C6H14O6 — the stable polyol

Chemical name: (2R,3R,4R,5R)-hexane-1,2,3,4,5,6-hexol. Solubility ~22g/100ml at 25°C. Heat of solution -121 kJ/kg gives that signature cooling effect in mouth. Does not undergo Maillard browning — perfect for heat-stable pharma and sugar-free coatings.

  • Osmotic: raises plasma osmolality, draws water across membranes
  • Inert: chemically unreactive excipient, ideal diluent
  • Stable: non-hygroscopic, melting 165-170°C

02 — How it works

Osmosis, metabolism & taste

Osmotic effect laboratory

Brain & kidney osmosis

IV mannitol stays extracellular, increases blood osmolality to ~300-320 mOsm/kg, pulls water from brain parenchyma, reduces intracranial pressure in 15-30 min. In tubules it blocks water reabsorption — osmotic diuresis.

Metabolism and glycemic health

Slow metabolism

Only ~20-30% absorbed in small intestine, slowly converted to fructose in liver. Result: 1.6 kcal/g, GI 0-2, no insulin spike. Excess ferments in colon — hence laxative threshold ~20-30g/day.

Sweet taste crystals

Cool sweetness

Negative heat of solution creates cooling. 0.5-0.6x sucrose sweetness, clean aftertaste, synergistic with sucralose, stevia, xylitol. Does not caramelize — keeps white coatings snow-white.

03 — Applications

Where mannitol hides

💊 Pharma excipient

60% of pharma mannitol goes to chewable, orodispersible and lyophilized tablets. Pearlitol, Mannogem grades give hardness + fast disintegration + pleasant mouthfeel. Also lyoprotectant for vaccines and biologics.

🏥 Hospital osmotic

20% IV for raised intracranial pressure, acute glaucoma, rhabdomyolysis diuresis, cisplatin nephroprotection. Inhaled mannitol (Bronchitol) hydrates cystic fibrosis airways and tests bronchial hyperresponsiveness.

ICPCF therapy

🍬 Food E421

Sugar-free gum, mints, chocolate coatings, diabetic jams, breath fresheners. Prevents sticking, gives crunch, no browning. ADI not specified — JECFA/ EFSA consider safe at quantum satis with laxative label >10%.

Dosage & safety snapshot

Food: 5-15g per serving typical, label warning if excess may have laxative effect. Medical IV: 0.25-1 g/kg test dose, up to 2 g/kg. Inhalation: 400mg BID for CF. Contraindicated in anuria, severe heart failure, pulmonary edema. Always under medical supervision for IV use.

05 — From seaweed to sachet

How mannitol is made

1. Source

Fructose syrup from corn/wheat starch or Laminaria seaweed extract rich in mannitol.

2. Hydrogenate

Catalytic hydrogenation at high pressure with Raney nickel → 50/50 mannitol + sorbitol mix.

3. Crystallize

Fractional crystallization separates low-solubility mannitol. Multi-stage recrystallization to 99%+.

4. Mill & grade

Spray-drying, granulation, sieving, sterile filtration for IV. QC: purity, endotoxin, PSD.

Mannitol production facility

Sustainability note

Modern plants recycle nickel catalyst, use membrane filtration, and valorize sorbitol co-product. Seaweed route is low-land, CO2-absorbing. Mannitol itself is biodegradable and non-ecotoxic. Look for non-GMO corn, RSPO-free, ISO 14001 suppliers.

06 — FAQ

Questions, answered

Is mannitol safe?

Yes. FDA GRAS, EFSA E421, JECFA ADI not specified. Safe in food at normal levels. Large doses >20g can cause bloating and laxative effect — that is normal osmotic action, not toxicity.

Is mannitol sugar? Can diabetics use it?

It tastes sweet but is not sugar. GI near zero, 1.6 kcal/g, minimal insulin impact. Widely used in diabetic sweets. Count half carbs as per dietitian advice.

Is mannitol keto / vegan?

Vegan, gluten-free, non-animal. Keto-friendly in small doses (gum, mints) but subtract polyol carbs carefully — excess can affect ketosis via gut fermentation.

Why is mannitol given in hospitals?

To lower brain swelling, eye pressure, and to force diuresis to protect kidneys. Only IV under monitoring — it shifts fluids and electrolytes powerfully.

Mannitol safety and dosage

Storage cheat-sheet

Store sealed below 25°C, RH <65%. Non-hygroscopic so no clumping, shelf life 3-5 years. Keep IV bags from freezing (crystallization reversible by warming). Food products: declare E421, add “excessive consumption may produce laxative effects” if >10% polyol.
